Record Keeping for Contact Tracing

The safety and well-being of Mid Valley’s customers, retailers and employees continues to be of greatest importance.  As per the Covid-19 requirement for seated dining, Mid Valley must collect and keep records (for 28 days) of customers that dine in the centre’s food court precinct.

Please complete the following details before being seated:

When visiting Mid Valley customers are required to; practice social distancing, wear a face covering, exercise a high standard of hygiene and to wash their hands thoroughly and to please stay at home if you are unwell.
